چکیده

I n this issue, Yamaji et al 1 report the very long-term (15-year) outcomes of patients treated successfully with balloon angioplasty (BA) compared with bare-metal stent implantation (BMS). Considering the major progresses made in interventional cardiology since the advent of BA, it would be reasonable to question what relevance these data have in the era of thirdand fourth-generation drug-eluting stents (DESs) that have biodegradable polymers or are polymer free or have a completely bioabsorbable platform. This report comes at an opportune moment in interventional cardiology because of the emergence and availability of the ABSORB everolimus-eluting bioresorbable vascular scaffold (BVS; Abbott Vascular, Santa Clara, CA). These data might be our “crystal ball” into predicting late and very late events after nonpermanent stent alternatives such as drug-eluting balloons (DEBs) or BVS (the BA group) or after implantation of a DES with biodegradable or no polymers (the BMS group). In the current study, Yamaji et al report the very long-term outcomes of patients who were free from early restenosis. The main findings were the following:

Coronary and endovascular applications of the Absorb TM bioresorbable vascular scaffold CliniCal trial report

Atherosclerotic cardiovascular disease is the leading cause of death and disability in the world, accounting for nearly one-third of all human mortality. In the modern era, the mainstay of therapy for established occlusive lesions in the cardiovascular tree is percutaneous balloon angioplasty and stent implantation. The procedure is widely applied, with over 2 million procedures performed annua...

Current status of bioresorbable scaffolds in the treatment of coronary artery disease.

State-of-the-art drug-eluting metal stents are the gold standard for interventional treatment of coronary artery disease. Although they overcome some disadvantages and limitations of plain balloon angioplasty and bare-metal stents, some limitations apply, most notably a chronic local inflammatory reaction due to permanent implantation of a foreign body, restriction of vascular vasomotion due to...

Macromolecular Approaches to Prevent Thrombosis and Intimal Hyperplasia Following Percutaneous Coronary Intervention

Cardiovascular disease remains one of the largest contributors to death worldwide. Improvements in cardiovascular technology leading to the current generation of drug-eluting stents, bioresorbable stents, and drug-eluting balloons, coupled with advances in antirestenotic therapeutics developed by pharmaceutical community, have had a profound impact on quality of life and longevity. However, the...

From metallic cages to transient bioresorbable scaffolds: change in paradigm of coronary revascularization in the upcoming decade?

Drug-eluting bioresorbable scaffolds (BRSs) may in the near future change drastically the landscape of percutaneous coronary revascularization. In 1977, when Andreas Gruentzig introduced the concept of percutaneous transluminal coronary angioplasty, the most feared enemy of the operators was acute occlusion of the dilated lesion due to a combination of elastic recoil and intimal and medial diss...

Immediate and Short-term Follow-Up of Aortic Coarctation Balloon Angioplasty and Stenting

Background Aortic Coarcatation (CoA) is one of the congenital heart diseases with the rate of 5-8% of Coronary heart diseases(CHDs). Balloon angioplasty is now one of the effective way of treatment for CoA, native or Re-coarctation (Re-CoA). We aimed to assess the immediate, and short term response to angioplasty and stenting, and also complications. Materials and Methods Balloon angioplasty wi...
